At least one lieutenant pack is necessary for any Overlord playing Descent. The game is more than a bit unbalanced in favor of the heroes and these provide more options for the Overlord that might give him a bit of a fighting chance. These lieutenant packs contain a deck of cards, tokens, and a plastic miniature. The deck of cards provides the Overlord with some additional options by spending tokens that he earns through adventures. This eventually swings back in the favor of the heroes a little bit because after spending these tokens they are given to the heroes to use for small bonuses. This is the lieutenant pack I'm using for my current campaign. The theme of this deck is to provide bonuses and additional powers to bigger base monsters. I like using larger monsters because they typically have higher survivability so this fits really well into my play style. Plus the figure is a gorgeous dragon.

Miniatures for the bosses rather than tokens plus more tactical choices for the overlord make these a must have.
